Ambulatory Care

Works with community services, step up/down units, rapid response team, community rehabilition team, acute colleagues and A&E to assess patients to prevent admission to the acute trust where safe to do so.

Who is the service for?

For patients that have been referred by a professional for further assesment / admission to hospital, self presented to A&E or attended an outpatients appointment with symptoms that could be managed in primary care.

Community Rehabilitation Services

For patients who need to be seen in a non-hospital environment, e.g. their own homes, rehabilitation beds, and care homes. Assessment and treatment following fractures, strokes, surgery and building up confidence, strength and stamina in order to increase and maintain independence.

For anyone over the age of 18 who has been assessed as having rehabilitation needs and has consented to being referred to us

NHS111 non-emergency number. It’s fast, easy and free. Call 111 and speak to a highly trained adviser, supported by healthcare professionals. They will ask you a series of questions to assess your symptoms and immediately direct you to the best medical care for you.

Anyone who doesn't have a GP who requires non emergency care.

Night Nursing

Unsheduled care nursing service 23.00 - 08.00

Who is the service for?

For GPs or health professionals who require urgent advice.

Out of Hospital Care

Multidisciplinary team of nursing and therapy staff, supported by Social Workers, Pharmacist, Health and Social Care Coordinators.

People aged 18 and above in the Carlisle, Longtown and Brampton area 24/7
