Early Supported Stroke Discharge Team

Work with people who have had a stroke, family and carers, to help ensure a smooth, supported, early transition from hospital to the individual’s preferred community home environment.

Who is the service for?

People being discharged from hospital following a stroke who can transfer from chair to chair independently or with assistance of one family member/carer.

Heart Failure Team

The team carry out assessment and treatment with specialist Heart Failure nurses.

With conditions as described above.

Hospice at Home

Hospice at Home Carlisle and North Lakeland is a registered charity providing a free nursing service.

Who is the service for?

For people at the end stages of life who prefer to be cared for in their own homes.

Lymphoedema Service

Advice and support to patients with lymphoedema and chronic oedema.

Who is the service for?

Patients with a cancer diagnosis or end of life care.

Palliative Care Nursing

Offer advice on pain and symptom control, psychological and spiritual support and advice to families and carers.

Who is the service for?

For adults.

TB Nursing Services

Identifies cases of TB, support people receiving treatment, screen family and others who have come into close contact with TB patients.

Adults and young people, at risk groups.

Tissue Viability Services

Advice and treatment on chronic or hard to heal wounds.

Who is the service for?

For adults with specific medical requirements.
