Step Up Step Down

Aims to reduce unnecessary admissions and to give people the support they need to remain independent.

Patients who require short-term 24 hour nursing help or extra nursing to facilitate discharge.

TB Nursing Services

Identifies cases of TB, support people receiving treatment, screen family and others who have come into close contact with TB patients.

Adults and young people, at risk groups.
