Persistent Physical Symptoms Service (PPSS)

Provide evidence based interventions to patients with persistent symptoms of longer than 6 months, rather than due to any underlying pathology which includes chronic pain, Chronic fatigue syndrome, functional neurological disorders and medically unexplained symptoms.

Who is the service for?

Adults over 18, with persistent physical symptoms.

Carlisle Community COPD Team

Multidisicplinary team to help ensure COPD patients are recieving optimum treatment and support in management of their condition.

Who is the service for?

Patients with COPD.

Community Rehabilitation Services

For patients who need to be seen in a non-hospital environment, e.g. their own homes, rehabilitation beds, and care homes. Assessment and treatment following fractures, strokes, surgery and building up confidence, strength and stamina in order to increase and maintain independence.

Who is the service for?

For anyone over the age of 18 who has been assessed as having rehabilitation needs and has consented to being referred to us

Cumbria Community Acquired Brain Injury Rehabilitation Team

Community inter-disciplinary team providing time-limited rehabilitation to adults (aged 16 and over) following acquired brain injury (ABI.)

Who is the service for?

The injured person (adults aged 16 and above), their family, health and social care professionals.

Early Supported Stroke Discharge Team

Work with people who have had a stroke, family and carers, to help ensure a smooth, supported, early transition from hospital to the individual’s preferred community home environment.

Who is the service for?

People being discharged from hospital following a stroke who can transfer from chair to chair independently or with assistance of one family member/carer.

Neurology Services - Physiotherapy

Provides physiotherapy assessment and treatment for anyone over 18 years with a neurological diagnosis.

Occupational Therapy

Help people engage as independently as possible in the activities (occupations) which enhance their health and wellbeing.

People who have difficulty with everyday tasks because of a physical disability, learning disability, mental health condition or medical condition.

Physical Health and Rehabilitation Psychology Service

To support people with long-term conditions to manage the impact of the condition upon their life.

Who is the service for?

For people who are having difficulties managing their long term condition and people who have developed a secondary psychological problem due to the events surrounding living with a long term condition.

Physiotherapy- Extended Scope Physiotherapy Services (ESP)

Physiotherapy specialists who work beyond the traditional scope of physiotherapy practice.

Who is the service for?

Complex musculoskeletal conditions unlikely to require surgery or Consultant’s advice.

Physiotherapy- MSK Physiotherapy Services

Comprehensive orthopaedic assessment, treatment and advice on management of musculoskeletal conditions.

Adults with musculoskeletal disorders/injuries.

Psychological Services Long Term Conditions

Aims to offer specialist neuropsychological assessment, rehabilitation and advice for patients.
