DESMOND - Diabetes Education and Self Management for Ongoing and Newly Diagnosed.

Who is the service for?

Adults with Type 2 Diabtes

Diabetes Prevention Programme

Programme to support people to reduce risk of developing Type 2 Diabetes.

Who is the service for?

Adults at risk of developing Type 2 Diabetes with an HbA1c of 42-47mmol/L

Diabetic Foot Clinic

Multidiscplinary team improves outcomes for patients with Diabetic foot disease. Input from Vascular, Diabetes, Podiatry, Orthopedic and Orthotic clinicians.

Who is the service for?

Criteria for referrals are new foot ulceration, static or non-healing ulcreation, extensive ulceration/tissue loss, spreading infection/unresolving cellulitus, Osteomyelitis, Gangrene, critical limb ischaemia, unexplained hot, red, swollen foot with or without pain, suspision of acute Charcot arthropathy.

Paediatric Dietetics

Works with children with allergies, weight management, nutritional support, IBD, coeliac disease, borderline anorexia and others.

Who is the service for?

Children up to 18 years of ages. Also provides dietary support for children, families and carers.
